TO LIGHT A FIRE ON THE FOLLOWING DATE, TIME, AND LOCATION

AS PER THE FOLLOWING CONDITIONS

1. The fire must only be ignited at the above noted location, date and time.
2. The fire must be constantly tended and supervised by a responsible adult.
3. No petroleum products are to be burnt.
4. This permit must be retained and presented upon request from a fire department officer, or law enforcement officer.
5. If and when directed by the fire department, the permit holder must immediately extinguish the fire.
6. The permit holder assumes full responsibility for the fire, and thereby assures the safety of onlookers and protection of surrounding property.
